VITAL STATISTICS Updated May 14, 2008

The City of Highland Park preserves local records on births and deaths that occur within its district. Since birth and death records first became archived in 1916, thousands have been placed on file, including some delayed records dating back to the mid-1800s.

Individuals occasionally may need certified copies of birth or death certificates for such diverse purposes as applying for passports, fulfilling school entrance requirements, filing license applications, or seeking insurance or government benefits. These records are available only to the following:

  • Birth Certificates: Individual (18 and over), parent or legal guardian ONLY
  • Death Certificates: Public Record

Vital Records Fee
Birth certificates are $8.00 for the first certified copy, $4.00 for each additional certified copy. Death certificates are $12.00 for the first certified copy, $4.00 for each additional certified copy thereafter.

Certificates can be requested through the mail, by completing the attached application and submitting it to our offices.

Birth certificates can only be requested by the individual (18 and over) or a parent/legal guardian. If you are requesting the certificate via mail please include a photocopy of your state issued Photo ID along with the request form.

Death records are not public records and are only available to those with a personal or property right interest with the decedent.
